For patients desiring greater access, flexibility, and health maintenance counseling, we offer the following special plan (with fixed semi-annual payments). The plan includes:
Annual comprehensive physical exam (with Nurse Practitioner)
Up to 20 outpatient visits with Dr. Madan
Up to 20 hospital visits (at Sequoia Hospital)
Fee for service for extra visits
Comprehensive health management and preventive care
24-hour access for emergency care
House calls and phone consultations at the discretion of Dr. Madan
You may enroll in our Annual Plan in January and July of each year. Please call our office for more details.
